Deductible

The amount you generally pay for covered care before the insurance plan starts paying for many services.

Coinsurance

Your percentage share of covered costs after the deductible is met.

Out-of-pocket maximum

The most you should pay for covered in-network care during a plan year, excluding premiums and non-covered services.

Network

The doctors, clinics, hospitals, pharmacies, and other providers contracted with a plan.

Benefit period

The length of time a policy may pay benefits once eligible claims begin.

Elimination period

The waiting period before some disability or long-term care benefits begin.
